Licensed Practical Nurse

Details: Licensed Practical NurseIN ADDITION TO THE GENERAL PERFORMANCE WORK STATEMENT, THE FOLLOWING EXTENDED DESCRIPTION APPLIES. 1           Qualifications1.1          All Licensed Practical Nurses shall have and maintain current certification in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS). The contractor staff shall be responsible for obtaining and maintaining the necessary recertification.  While providing services under this contract the contractor staff shall maintain current certification in at least one state in the United States.  The contractor staff is required to comply with any applicable state laws concerning their profession.1.2          Each Licensed Practical Nurse shall:1.2.1          Possess a degree in nursing from an accredited school of nursing from at least one state or territory of the United States.   Have active unencumbered license from any state or territory of the United States.1.2.2          Have been continuously employed in the practice of Licensed Practical Nurse for the two years prior to the commencement of services under this contract.  2           Hours of Performance2.1          Normal Duty Hours.  Contract Licensed Practical Nurses shall perform services at Clark Health Clinic during a 9-hour shift, between the hours of  0630 and 2200, Monday through Friday (excluding Federal Holidays).  This shift includes a 60-minute lunch break, which is not paid by the Government.  Shifts will be assigned by the clinic Officer-In-Charge (OIC) based on the staffing needs of the clinic.  Schedules will be prepared and distributed 30 days before the beginning of the month the service is to be provided.  5.  Specific Tasks5.1. All task to be performed by the HCPs shall include but not be limited to the following.  5.1.1  All Licensed Practical Nurses provided by the contractor shall perform essentially the same functions for patients ranging from two weeks through geriatric, within the scope of acceptable nursing practice, as those required by Army or Government service health care professionals of similar experience and in similar duty assignments.  Practices and productivity will be compared to that of other like staff assigned to the same medical center.  The Licensed Practical Nurses shall attend and participate in patient nursing reports, patient care conferences, team/clinic conferences, discharge planning, and staff conferences as deemed appropriate administrative officials.  The duties listed below shall be applicable to all Licensed Practical Nurses as well as to the specific nursing specialties indicated. All Licensed Practical Nurses shall:5.1.2. All tasks to be performed by the HCPs shall include but not be limited to the following:   Obtain patient history and pertinent family history.  Implement a plan of instruction and document demonstrated level of understanding.  Target age specific needs.5.1.3.  Be aware of potential or life threatening crisis as shown by physical or vital sign assessment.  Distinguish between normal and abnormal physical findings.  5.1.4.  Maintain composure and react appropriately while relating to patients families and staff; manage distraught, irritable, unreasonable or angry individuals and maintain positive rapport with all individuals involved in patient care.  5.1.5.  Act/react with alertness and skill in any emergency situation, e.g., cardiac or respiratory arrest, hemorrhage, shock, severe physical trauma and psychiatric reaction.  Coordinate with appropriate personnel for assistance in emergency situations.5.1.6.  Conduct limited interviews to facilitate triage to the appropriate level of care and coordinate patient flow throughout the visit.  Communicate and coordinate with appropriate personnel to assist in delivery of care where indicated.  Assist in arranging out-of and within-facility transfers of patients.  Communicate pertinent patient information to the charge nurse and/or physician as appropriate. Transcribe and verify transcription of physician orders.5.1.7.  Assist physician with making patient telephone contact, transport of medical records and medical documentation between sections, making copies of medical documentation, and other general assistance to facilitate efficient patient care.  5.1.8.  Assist physician with patient examinations, application of dressings and specimen collection.  Set up examination room, assist patient to disrobe/dress according to level of assistance and required drape, position patient and chaperone as necessary.  Prior to transporting specimens, label and document appropriate patient information.  5.1.9.  Administrate or assist with minor, acute, and routine tasks in accordance with SOP’s and training rendered under the supervision of a provider, nurse or other assigned personnel (12 Lead EKG, urinalysis dipstick, PAP/Pelvic examination, slide preparation for KOH/WET prep, and pap smears, hemoccult testing, throat culture, orthostatic vital signs, wound cleaning, phlebotomy, PKU screenings, heelsticks, daily quality control checks)5.1.10.  Provide nursing care and monitoring of patients.  Recognize arrhythmias and respond appropriately.  Operate EKG machine and cardiac monitor and have knowledge of the operation of a defibrillator.  Carry out prescribed physician orders and document in medical record.  Administer medications and blood products with the knowledge of actions and usual dosages of commonly used drugs.  Document patient response to administered medications.  Interpret lab values and perform blood drawing (i.e., heel stick for dextrostix, hematorcrits, bilirubins, complete blood counts, blood cultures, and blood chemistries).  Licensed Practical Nurses will not administer IV push medication or start blood product transfusions.   5.1.11.  Care for pre-term and post-partum patients. Recognize changes in maternal or fetal condition that would constitute an emergency situation and respond appropriately to the situation as per standards of care.  Teach patients and family members on family planning, infant and post-partum care.5.1.12.  Recognize normal growth and development for newborn to eighteen (18) years.  Care for pediatric patients to include:  vital and neurological signs; developmental assessments; fluid intake and output; psychosocial and physical indicators of potential complications; and family interactions.   5.1.13.  Clean and restock procedure and patient rooms with supplies, forms and linen, dispose of trash and soiled linen, supplies and equipment.  Exercise universal precautions set forth by the Centers for Disease Control when providing direct patient care and/or indirect patient care involving possible exposure to contaminated items.  Clean, disinfect and reassemble instruments in accordance with SOP
